Our goal at Performance Dental Center is to ensure that all our patients receive the quality dental care they need and deserve, and we understand that finances are part of that consideration. With this in mind, we work with most dental insurances, for those that do not have insurance or have limited coverage we offer an In House plan, and we also offer multiple financing options that are easy to apply for with approval in minutes.

We believe dental health is essential to successful physical and mental performance. We also understand that it can be costly and dental insurance can be confusing. Our goal with our PDC In-House Dental is to make your dental care straightforward and affordable. Our pricing is comparable to other Monthly Dental Plans without the monthly bill. Everything is paid upfront and you then get the benefits throughout the rest of the year!

What's Included for each individual person on the plan

  • 2 Complete Periodic Annual Exams and Unlimited Targeted Exams
    • Each person signed up for the plan will have 2 Complete Periodic exams and any limited exams you may need for targeted issues throughout the year as well.
  • 2 Annual Cleanings
    • Each person signed up for the plan will receive healthy mouth or periodontal maintenance cleanings each year
  • 1 Set of Yearly X-Rays and unlimited targeted xrays
    • We will take a full set of the necessary x-rays once a year and then any single x-rays you may need throughout the year for targeted issues.
  • 1 Fluoride Application
    • We will provide one Free Fluoride application a year. Each Additional Application is $35.00
  • 30% Discount*
    • You receive up to a 30% discount on all other services you may need. Additional cleanings, fillings, crowns, implants and more.
    • *Invisalign and Orthodontics are not included in the discount.

Dental Insurance

Performance Dental Center will bill out your insurance as a courtesy to you. Even if we are not considered an in-network provider we will still send out your claim. Here are a few things to remember regarding your insurance.
  • We ask that you provide us with the most up to date insurance information a minimum of 72 hours prior to your appointment.
    • You may do this by sending a us a copy of your card via text to (833) 209-3916 or via email to or by calling our office at (833) 209-3916.
    • This helps us in providing you with the best possible estimate for your patient responsible balance and prevent any unnecessary out of pocket costs to you on the day of your visit.
  • If we do not receive updated information prior to your appointment we cannot guarantee our network status, pre estimate your patient responsible benefits, and estimated out of pocket costs for that days visits.
  • Sometimes verifying your new insurance is quick and simple, other times it can be a time consuming process and we cannot guarantee that this will be completed during your visit
  • If we are unable to verify your insurance prior to or during your visit then we do require that the full cost of the visit is paid up front.
    • Once we are able to verify your insurance and they have made payment we will then provide you with an account credit or at your request a refund for any overpaid amount.
  • We are not “In-Network” with any DMO or DHMO dental plans. If you are not sure if your dental insurance is considered a DMO or DHMO please call the number on the back of your card or visit their website or app to find out if you are with a DMO plan. If you are ask them to provide you with a list of network providers as this is a smaller group of providers.

Please call us if your insurance is not listed here, there are many smaller companies with plans that will fall under the umbrella of these companies that we are considered In-Network with. We are happy to confirm network coverage for you. You may also call the customer service number on the back of your insurance card to verify network status.
